Patient Support and Care Navigator

The Support and Care Navigator assists the client along their journey through WellFort’s programs and services.  The SCN may be assigned to focus on one or more stages of the client’s journey, but generally they will have the capacity to support clients through intake, assessment and service delivery and provide instrumental and community support as needed, working within WellFort’s service delivery frameworks.

Community Health Ambassador

The Community Health Ambassador plays a key role in community health initiatives, advocacy, client engagement, population health while working within WellFort’s Open Focused Care service delivery. Community Health Ambassador will collaboratively with community groups, networking agencies and other stakeholders using a community development model.

Prevention Specialist

The Prevention Specialist, is responsible for developing, implementing, and evaluating Preventive Care Plan aimed at improving the overall health and well-being of the client and the community. This role is part of an integrated care team collaborating with other healthcare professionals, community organizations, providing health education, completed health assessments, preventive care planning, connection to resources, and interventions designed to prevent illness, injury.

Recruitment and Onboarding Specialist

We are seeking a Recruitment and Onboarding Specialist to join our team and play a pivotal role in shaping the future of our organization. As the first point of contact for new talent, you will lead full-cycle recruitment and deliver an inclusive and seamless onboarding experience that embodies our organization’s values. This role is expected to be in the office 1-2 days per week.

A key aspect of this role is providing expert leadership and guidance in procurement and contracting processes. This position contributes to strategic financial decision-making and helps streamline procurement activities, enhancing the organization’s overall financial efficiency and effectiveness.
