The Overnight Staff role involves monitoring the facility and residents, ensuring safety and security during the night. Key responsibilities include conducting regular attendance checks, responding to incidents, ensuring the building is properly locked and alarmed, and completing detailed reports.
The CHW – Outreach – East Toronto Outreach Project (ETOP) will engage and work with community members who use drugs and have mental health issues through the provision of outreach, group and support services/resources to support individuals in accessing and maintaining connections with mental health, primary care and harm reduction programs/services.
The Community Health Worker – SHHR will engage and work with community members who use drugs and have mental health issues through the provision of short-term case management, overdose prevention education, harm reduction supply distribution, harm reduction education provision and through referrals to additional services.
The Support and Care Navigator assists the client along their journey through WellFort’s programs and services. The SCN may be assigned to focus on one or more stages of the client’s journey, but generally they will have the capacity to support clients through intake, assessment and service delivery and provide instrumental and community support as needed, working within WellFort’s service delivery frameworks.
The Community Health Ambassador plays a key role in community health initiatives, advocacy, client engagement, population health while working within WellFort’s Open Focused Care service delivery. Community Health Ambassador will collaboratively with community groups, networking agencies and other stakeholders using a community development model.
The Prevention Specialist, is responsible for developing, implementing, and evaluating Preventive Care Plan aimed at improving the overall health and well-being of the client and the community. This role is part of an integrated care team collaborating with other healthcare professionals, community organizations, providing health education, completed health assessments, preventive care planning, connection to resources, and interventions designed to prevent illness, injury.
The Nurse Practitioner (NP) works with 2 focused areas of service including providing primary health care for a roster of clients at the centre and providing primary health care for outreach clients.
